To sign up for our DPC membership, please click the button below. It will take you to our forms section to complete your enrollment.

For patients who are not current patients of Dr. Winston through Amanda Winston MD PLLC, there will be an additional nonrefundable enrollment fee of $100 (per patient), which will be automatically charged to your card within one week of enrollment.

Once you have completed your enrollment, our office will reach out to confirm receipt of your application and status as our patient.
